Facility User Agreement - Minor



This agreement provides Parks and Recreation staff needed patron information, particularly emergency contact information, as well as an acknowledgement of rules and inherent risk by facility users and Safe Child Policies by parents/legal guardians of minors.
This is to be completed by any non-member visitors.

Note: Parents/Guardians must complete the attached Safe Child Acknowledgment Form. Children under 10 are not allowed in the facility without a parent or guardian over 16 supervising them.
